Medical: Blue Cross Personal Choice Healthcare

Effective first of the month, 30 days from date of employment. The health plan options include prescription medication coverage. You may cover yourself, spouse, and dependent children up to age 26. Working spouses with medical benefits available to them through their employer are not eligible to participate in ASI’s insurance plan.

High Deductible Health Plan (HDHP) with Health Savings Account (HSA):

The Personal Choice HDHP allows you to save money with lower biweekly deductions, in addition to enrolling in the Health Savings Account (HSA). Preventive services are fully covered. A Health Savings Account is a savings plan that allows you to set aside tax-free contributions to use for eligible health care expenses, including your deductible and coinsurance. Money left unused at the end of the year will be rolled over to the next year. ASI contributes $750 for single coverage or $1,500 for adult/spouse, adult/children, or family coverage to your HSA Account. Under this plan, the prescription program is administered through Blue Cross. When filling a prescription, you will pay the Blue Cross discounted rate upfront until your annual deductible has been met. Once the deductible has been reached, only prescription co-pays will be charged until you reach your out-of-pocket maximum. This plan contains a Preventive Drug coverage, which includes specific scripts subject to only to the copay amounts, rather than a full upfront payment.

Basic & Buy-up Plans/Express Scripts Rx:

Blue Cross Personal Choice coverage is available for an employee payroll contribution as low as $79.83 bi-weekly.  You’ll have two plan options to choose from either the (1) Personal Choice Basic Plan or the (2) Personal Choice Buy-up Plan.  Both are easy to use medical plans which do not require you to choose a primary doctor and referrals are not needed.  The Basic Plan includes in-network co-pay of $20 to visit your primary doctor and $40 to visit a specialist with a yearly deductible of $1,000 per person, $3,000 maximum for family, regardless of the number of eligible dependents. The Buy-up Plan includes in-network co-pays of $25 to visit your primary doctor and $40 to visit a specialist with a yearly deductible of $1,000 per person, $2,000 maximum for family, regardless of the number of eligible dependents.

MetLife Dental, Vision, Accident, Critical Illness and MetLaw Insurance Options

These optional insurance elections are effective the first of the month, 90 days from date of employment.

DENTAL: Diagnostic procedures (bi-annual visits) that include exams, x-ray and teeth cleaning are covered 100%. Other services such as root canal, fillings, treatment of gum disorders, crowns, dentures and bridgework are paid at a percentage (50%-80%) and are subject to an annual deductible ($50 per person, maximum of $150 per family). The choice of dentist is left up to the employee.

VISION: Benefits for a wide range of covered services, including: Ultraviolet (UV) Coating, Polycarbonate (shatter resistant) lenses for children and fixed copays on popular lens options. Includes access to thousands of private practice providers and top optical chains like Costco Optical, EyeMasters, Visionworks, RX Optical and more, with out-of-network coverage also available. 

ACCIDENT: Guaranteed coverage on over 150 events, including injuries, hospital treatments, medical services and dismemberment/paralysis. Coverage includes direct payments that allow you to spend as you choose and options to continue coverage if you change jobs or retire.

CRITICAL ILLNESS: Guaranteed wide range of coverage on illnesses including cancer, Alzheimer’s Disease and major organ transplant (pre-existing illness excluded). Offers lump sum payments upon diagnosis verification without waiting periods or age restrictions. Includes recurrence benefit and no pre-existing exclusion for heart attack or stroke. 

METLAW: Access to experienced, credentialed network attorneys in person or by telephone for full service on an unlimited number of some of the most common personal legal matters. Plan includes no deductibles, claim forms, copays or usage limits. 

Life Insurance

The effective date is the first of the month, 60 days from date of employment.

Group life insurance is paid in full by ASI for all regular full-time employees. This benefit includes accidental death and dismemberment in the amount equal to the employee's average annual salary, not to exceed $300,000. Managers and supervisors are eligible to receive 1.5 times their salary to a maximum of $300,000. Benefits will reduce by 35% at age 65 and reduce by 50% at age 70.

Disability Insurance

The effective date is the first of the month, six months from date of employment.

Non-salaried employees may be granted a short-term disability leave of absence (due to an approved disability) for a maximum period of up to 13 weeks.  The non-salaried employee will receive partial compensation during that time, 60% of their base rate up to $1,000.00 per week.  This plan excludes pre-existing conditions that occur within the three months prior to the effective date of coverage. Short-term disability is offered to all FT/non-salaried employees at no cost to the employee.

All employees may be eligible for long-term disability that pays up to 60% of the employee's monthly earnings (up to a max of $5,000.00 monthly for hourly employees, and a max of $10,000 monthly for salaried employees).  These payments begin after 90 days of total disability. This plan excludes pre-existing conditions that occur within the three months prior to the effective date of coverage.  During the waiting period, the salaried employee may receive up to 100% of their average salary (one month for each full year worked). Long-term disability is offered to all full-time employees at no cost to the employee.

Cohn Family 401 (k) Matching Plan

Employees may payroll deduct, pre-tax, up to 85% of their total gross yearly earnings, up to the allowable maximum by the IRS, and choose from several investment options.

ASI pays the entire cost of the 401(k) program with a maximum contribution equal to 3.5% of the participant's yearly income. Employees will be eligible to receive the company match after completing one year and a minimum 1000 hours of service during the plan year, and after being actively employed by ASI on the last day of the year (December 31st). An employee's match will be calculated after the first quarter of the next year and is subject to a six-year vesting schedule (2 years is 20%, 3 years is 40%, 4 years is 60%, 5 years is 80% and 100% vested at 6 years.)

Paid Time Off (PTO)

Earn PTO at the end of each full month of work to be used for vacation, personal or sick days:

  • 0-5 years: 17 days
  • 5+ years: 22 days
  • 15 years: 27 days
  • 25 years: 32 days

Paid Holidays

All regular full-time and part-time employees are eligible for the following paid holidays, provided you work your full schedule the day before and after the holiday.

Tuition Reimbursement

Tuition reimbursement is available for several educational needs, including: college degrees, professional certification, workshops/seminars and scholarships. Employees may receive reimbursement for all approved job-related courses taken at an accredited technical school or college.

On-Site Daycare

ASI provides on-site child care at affordable rates to employees and their families. The fully-accredited Lots of Love Learning Center began in 1986 and has a qualified staff of twelve child-care professionals who specialize in caring for children of various ages. The daycare offers full-day, half-day and before and after school programs. New parents (who are ASI employees) are offered eight weeks of paid daycare to use within the first six months of returning to work.
