Aetna Choice POS: Effective date is the first of the month, 30 days from date of employment. Employees are responsible to contribute a portion of the health insurance premium via a pre-tax payroll deduction.
Aetna Choice POS allows you to select your own doctors
and hospitals. Coverage includes: hospitalization, office
visits, adult and child preventive care and a prescription
plan. You’ll have two plan options to choose from
either the (1) Basic Plan or the (2) Premium Plan. Under either plan you’ll
pay $20 to visit an in-network doctor and $35 to visit a specialist. No
referrals are needed to visit a specialist. If you visit an “out-of-network” doctor,
you will be responsible for a yearly deductible of $500 per person, $1000 maximum
for family. Aetna then pays 60% of eligible expenses with a yearly cap of $3,000
for individual or $6,000 for family, including the deductible, Aetna then pays
at 100% for the remainder of the plan year (January 1st through December 31st).
Aetna Choice POS includes a prescription program. When you purchase
prescription drugs at a participating pharmacy, you pay a $10 co-pay for generics,
a $20 co-pay for formulary and a $35 co-pay for non-formulary generic and brand
name. At a non-participating pharmacy you’ll pay for the prescription
and request reimbursement from Aetna Choice POS. You’ll be reimbursed at
the Aetna Choice POS approved amount allowed for that prescription. A 90-day
supply of medication is also available through mail order.
Dental
Delta Dental Insurance: Effective

Diagnostic procedures (bi-annual visits) that include
exams, x-ray and teeth cleaning are covered 90%. Other
services such as root canal, fillings, treatment of gum
disorders, crowns, dentures
and bridgework are paid at a percentage (50%-80%) and are
subject to an annual deductible ($50 per person, maximum

the employee.
Life Insurance
The effective date is the
first of the month, 30 days from date of employment.
Group life insurance is paid in full by ASI for all regular
full-time employees. This benefit includes accidental death
and dismemberment in the amount equal to the employee’s
average annual salary, not to exceed $300,000. Managers
and supervisors are eligible to receive 1.5 times their
salary to a maximum of $300,000. Benefits will reduce by
35% at age 65 and reduce by 50% at age 70.
Disability Insurance
The effective date is the first of the
month, six months from date of employment.
Non-salaried employees may be eligible for short-term
disability payments for a maximum period of thirteen weeks.
Short-term disability
offers the non-salaried employee partial compensation,
60% of their base rate, up to $500 per week. This plan
excludes pre-existing conditions that occur within the
three months prior to the effective date of coverage.
Salaried employees may be eligible for long-term disability
that pays up to 60% of the employee’s monthly earnings,
a maximum of $10,000 per month. These payments begin after
90 days of disability. During this waiting period, salaried
employees may receive up to 100% of their average salary,
one month for each full year worked. This plan excludes
pre-existing conditions that occur within the three months
prior to the effective date of coverage
401 (k) and Profit-Sharing
Transamerica Pension administers our 401(k) savings plan.
Employees, 21 years of age and older, may enter the program
on the first of the month after completing six months and a minimum 500 hours of service. You may payroll deduct,
pre-tax, up to 85% of your total gross yearly earnings
($15,500 maximum) and choose from several investment
options.
ASI pays the entire cost of a profit-sharing program with
a contribution equal to 3% of the participant’s yearly
income. Employees, 21 years of age and older will be eligible for the profit sharing plan after completing one year and a minimum 1000 hours of service. Although ASI is not required to make a full 3%
contribution each year, we have yet to miss making that
contribution. Your profit-sharing fund will fully vest
